I reckon this is created using Photoshop and you've used 2 photos to create this and you've used a smudge tool on the background.

My guess is the background is created from the bokeh (blur/out of focus) of the lens by using a long-range zoom such as a 70-200 and an open aperture such as f/2.8 which blurs the background, with the focus on the subject at 200 mm and there is a…
